Numbers are drawn at the foot of each page when the document is saved, and they follow the order in the grid rather than the order the pages arrived in. Reorder first and the numbering agrees with the finished document — which is the whole reason to number after assembling rather than before.
If you merged several files, numbering runs continuously across all of them, so a report built from four separate PDFs reads as one document rather than restarting at each seam.
The numbers are drawn into the page itself, in a standard font that every reader already has, so nothing is embedded and the file barely grows. Pages you rotate are numbered the right way up regardless, because the numbering is applied last — after the order and the rotations are settled.

Yes. Numbering is the last thing applied, so it always matches the order you can see in the grid.
Can I start numbering from something other than 1, or skip the cover?
Not yet. Both are reasonable and neither is built — the current version numbers every page from one.
